To make it as easy as possible for our league members, we have implemented an online form to automatically fill out and digitally sign the Medical Release Form. This form needs to be filled out for each participant, each year. After filling out the form, you will receive a completed and filled out Medical Release Form PDF via email. After receiving your PDF, please upload it to your child's Participant Info. Please also print a copy of the form and turn it into your coach, even if you have scanned it into your account for this year. All coaches need a hard copy on hand at all activities, in case there is an emergency. To upload the Medical Release Form to your child's Participant Info, please follow the instructions on this page.
